BOX TRUCKS/HOT SHOTS
***MUST PROVIDE DIMENSIONS AND WEIGHT CAPACITY FOR EACH UPON APPLICATION

1. OPERATING AUTHORITY LETTER
2. MOTOR CARRIER (MC) PERMIT NUMBER
3. DEPT. OF TRANSPORTATION (DOT) NUMBER
4. CERTIFICATE OF INSURANCE
*** MINIMUM $750,000 LIABILITY AND $100,000 CARGO ***
